Individual Author Contributions

List each author in the order they will appear on the manuscript. In the Contributions column, enter the relevant codes from Section 3 separated by commas (e.g., CON, MET, WRI, REV). Each author must also provide an ORCID iD or institutional ID and sign below.

If your manuscript has more than 6 authors, attach an additional sheet using the same format. Note: Case reports and technical notes are limited to 4 authors; other articles are limited to 6 authors. Additional authors require written justification and approval from the Editor-in-Chief.
